When you Render out the clip from After Effect or if you just keep the After Effects project in PPro you can put the Dip to White transition back on. You will get a more natural dip to white look if all the layers ( green screen footage, background and anything else like a vignette) are in one track. If you want the background to always be the same and just have the green screen clip come on and off I would recommend you check out the Ultra Keyer in PPro. It is actually really good.

That sounds strange. Is the image RGB and not CMYK? Is it 8 bit?

Check out the Reduce Banding filter and see if it does the trick. Go to Effects, Video Filters, Image Control : Reduce Banding.

I would play around with the Puppet Tool and see if it does the trick. It is amazing what it can do.

It is a Full Motion Video File. I have never worked with it before because it is used for video games. Are they sure they gave you the right codec and they didn’t mean something like FLV? I would make sure they didn’t mean FLV.

It depends on the angle. Remember, less is more. I think it looks fake on a lot of tutorials because they use to much blood. If the angle was the persons back and they where hitting the wall you really wouldn’t see much because their head would be blocking the blood left on the wall.

I think it was done in After Effects. I watched part of it frame by frame and when the camera flies left the microphones position stays 100% the same in relationship to the singer. If it was done with a real camera you would have parallax.
It could be a combo of in camera effect and using After Effects.
