There is a bug logged on the issue and we’re working on a fix with Nvidia.
Luke – If the other solutions didn’t do the trick you can also temporarily remove the GeForce GT 650M from the CUDA supported card list.
Also, do you have steps to how you got the program into this state?

You’re correct. Using a card from AJA, Blackmagic or Matrox will do the job you’re looking for.

Hi Samuel,
I’ve ran test with both the T-Tap and UltraStudio 3D. You will get best results when the video and audio are routed to the same device.
From Premiere’s preferences> Playback > Choose the same I/O card in Audio Device and Video Device.

Can you try checking to see if the plugins made it to the correct location. They should be in the MediaCore folder.
/Library/Application Support/Adobe/Common/Plug-ins/CS6/MediaCore/

Is the crash switching between applications easily reproduced? Can you tell me which applications your going between and any other steps you think might cause the crash.

Yes this is a known issue. For the workarounds
From FCP:
Keyframe the audio in FCP and it will retain the audio levels when opened in Premiere.From Premiere:
The audio is set to -infinity on import so you will have to manually adjust the audio there.Hope this is helpful,
