Check your sequence settings (Sequence>Sequence Settings).

  • Could you have the work area bar over just part of your timeline?

Sequence settings/preset should match the properties of your source video, especially resolution.
You can export as anything once the edit is done.

  • Sequence settings AND source file properties should match, especially resolution. You can undo the preference to scale media to fit screen, by right clicking on the clip in the timeline and deselecting “Scale to frame size”.

You can also export as Microsoft uncompressed .avi in either 24 bit or 32 bit if you have an alpha channel.

Could be a preview issue; change the zoom of the Program window from Fit to 100%.

Mp3’s have been problematic as source audio….some work, some dont. Suggest converting to PCM audio using Soundbooth or other app.

First, XP64 is not a recommended OS for CS4 by Adobe.

The Cineform files are very high bitrate and require fast hard drives. The size of your hard drives may indicate they are older/slower IDE drives, not SATA. I assume your Cineform files are on the 500 GB drive which may be badly fragmented and/or over 75% full which can slow things down further.
Suggest adding another 1 TB SATA drive.

Premiere is interpreting your footage wrong. The Interpret Footage dialog allows you to change the PAR to 1.2…for use in a Widescreen Sequence preset.

For stereo clips with only one channel, use the Fill Left or Fill Right audio effects to duplicate the one good channel.
Working with discreet stereo, one method could involve selecting the clip in the Project window and choose Clip>Audio Options>Break out to mono…creating 2 mono clips that can be panned by adjusting their volume in the timeline.
Another method involves the Audio Mixer which can write pan keyframes in real time…but beware, these keyframes are in the track not on the clip, preventing further editing.
