While I doubt any other app will recognize Premiere’s chapter markers, most DVD authoring apps will let you put in chapter markers, just make note of the times from Premiere’s timeline.

Because of a delay between desktop and DV audio playback you can have one or the other, not both. From the wing menu in the Program window, choose Playback Settings. Then select “External Device Audio” and select even if currently selected “DV 29.97i (720×480)”.

The “CTI” or Current Timeline Indicator moves at 1 frame at a time unless you select “Show audio time units” in the wing menu of the sequence. Then you can have movement to as little of 1/48,000th of a second.

“Blend frames for smooth motion
Motion in a clip may appear jerky when you change the speed of a clip, or output to a different frame rate. Make sure that frame blending is on to create new interpolated frames that smooth the motion. Frame blending is turned on by default.Choose Clip > Video Options > Frame Blend.”
OR turn it off!

You dont say what kind of camera you have but footage captured via firewire is simply data transfer with no changes made during capture. I would update your video card drivers on the MAC side as it is likely just a display issue.

AVCHD requires much horsepower to decompress. Minimum 2.6 Ghz Quad core and lots of RAM.

Have you added any 3rd party plug-ins to Premiere? If so, remove them from Premiere’s plug-in folder and see if that helps.

I can think of 2 workarounds;
1. Insert 10 seconds of black video at the beginning of your sequence.
2. Render the timeline and then manually start the camera recording and play the timeline out. -
The only QT codec worth using is h264. H264 is fine for most export targets from small web video to Blu-Ray. You will have control over bitrates and quality.
The only reason I can think of not to use H264 would be cross platform editing.
