Also, beware if your frame size in Live Type does not match your timeline (e.g. 720 x 486 vs. 720 x 480). This will soften your graphics.

You output to tape (at 29.97) using the Kona and then recapture.
You can do it in software using Compressor, but the encode time is lengthy.

Leave it 16:9. All DVD players do letterboxing internally.
Leave your frame rate 23.98. All DVD players add pulldown where necessary.In Compressor, set the resize quality in the Frame Controls to Better or Best.
Also, for a spot, do your MPEG 2 encoding using CBR (constant bit rate) instead of VBR (which the misleadingly named “best quality” preset uses)Use a bit rate of 7. Going any higher on a DVD-R is risky…some DVD players may choke.
Be sure also to encode your audio to AC-3.You’ll probably need to make your own preset for this.

If you download the latest Avid Codecs, you can use Compressor to convert them to Avid DNX

If output to tape w/Kona card is not an option, Compressor’s retiming controls in the Frame Controls can do this.

No. But your broadcast video monitor must support 16:9, if not your image will appear stretched (but normal in FCP)

No. Not if you have the correct settings all the way through (that means your DVD Player, your widescreen monitor, and your compression settings in Compressor).

Is the old Avid a Mac or a PC?
Does that version of MC have the setting for PC compatible file names switched on? Are the file names lengthy or do they have odd characters in them?
Grasping at straws here…if it opens in AE and QT player, it should work in Avid…so its probably something in the Avid thats causing this.
