Create a new sequence with the desired dimensions and use the Position or Attach Point settings in the Effect Controls panel to adjust the vertical center of your footage. Create keyframes at the beginning and end of each shot (or cut the video with the razor tool) if you would like to change the positioning at different points in the video.
Note that 803 is not a good number for encoding, especially if you plan to use H.264. Use an even number instead, and preferably one that is divisible by 16; 804 will work, 800 or 816 would be better.

There are several subtleties but the basic methods are straight-forward:
– Insert the clips on different tracks in the same sequence.
– Scale the size with the Motion effect.
– Trim image dimensions with the Crop effect.
– Set the Position to locate and animate each clip as desired; consider the video frame to be a grid with coordinates (0,0) in the upper left corner and (w,h) in the bottom right corner.It’s best to just spend time trying out different techniques.

Just checked in CS6 and it appears the full screen monitor cannot be toggled while working with two screens; it is either always on or always off.

In older versions of Premiere Pro by you could do this by going into Project Settings > General > Realtime Playback > External Device. I am not sure about the recent releases (can’t check right now).

The following functions can be used to change speed: Interpret Footage (works on an entire file), Speed/Duration (works on a clip), and Time Remapping (works on a portion of a clip). If one function does not meet your needs, two or more can be applied simultaneously to get a multiplier effect.
Alternatively, you could apply Speed/Duration and/or Time Remapping to a nested sequence that contained a clip with its speed already increased.

Create two windows–one for each monitor–instead of stretching one window across the two monitors. Just undock a panel, move it to the second monitor, and populate it with other panels as desired. Save the workspace under a new name if you don’t want to change the existing preset.
Alternatively, you can designate one of the displays as a full screen monitor using the Device setting under Preferences > Playback.

If the client uses QuickTime, try Sorenson or Sorenson Video 3. You might need to raise the black level or gamma to compensate for color shifting.

First select the monitor under Devices in Preferences > Playback. That should enable it in the sequence settings.
