As far as I’m concerned there HAS to be a keyframe, it’s just the way things work. I can’t think of a way of producing a transcode without one at the beginning.

That CODEC and a whole bunch of others are installed when you install Final Cut Pro. It’s not a free CODEC.

No, simply because H.264 is generally a delivery codec and as such is heavily compressed using all the tricks of compression to achieve high quality picture at a minimal data rate.
