Alas, for CS4 there is no drag your footage in the New Item icon to create a new sequence feature. This was introduce in CS5.
You will have to make a custom sequence. I do have to warn you CS4 does not work well with H.264 in a mov wrapper.

Templates are indeed the lower thirds etc. you use in the Titler.

Reinstall the lot.
I dont think you can install them seperately unless you have the download version.

I use the Advanced Frameserver.
I do not use it for AE but just tested it and it works.

Did the templates just disappear or did you never had them?

If you have the disks I would reinstall them all.
If that does not do the trick,
uninstall run Cleaner Tool and reinstall.

Import the sequences into one project and set them in one timeline in Premiere.
Export from Premiere or use DL to Encore.

You can import via DL different Premiere Pro sequences into Encore but setting them on one timeline does not always work well.

Did you turn i-Link-CONV off?
Othewise it will downsample.
