A bit rate calculator is a bit rate calculator. The data rate time the number of seconds is your file size. The problem comes into play when the data rate is variable. How would it be possible to determine in advance how tightly the video can be compressed?
So they are inaccurate. A little practice out to get you the experience you need to avoid the need for a calculator, no?

You need to fix your file associations. Just right click on the file in Explorer and select Open With, select Choose Program and find Premiere Pro after you check the checkbox that says “Always use the …”

I can’t imagine how fouled up your system would have to be to have Premiere Pro save Encore files. It just isn’t possible as far as I know unless you have some serious registry problems.
It is not that I don’t believe you, I just don’t think that Premiere Pro has the necessary code to save Encore files. It is more likely that your file associations in Windows have gotten fouled up.

Did you create a project with a stereo master track? Probably yours has a mono track instead?

Just add a title with the filename. Or, if you mean all of the file names of all of the clips, I don’t believe anyone has created a tool for that.
