Hi Richard,
It looks like your system is running off the Intel. Do a little research but here’s a few ways others have found to make this work.
– Nvidia control panel> manage 3d settings> Program settings, select the program then tell it to use nvidia instead of integrated graphics.
– Device Manager> Uninstall your GPU plus remove all the driver files then
reinstall and update your driver– From BIOS menu, change your primary graphics adapter to separate GPU
– Go into hardware manager and disable the Intel HD

311.10 appears to be a old driver. Can you please try 341.21 and let us know how it goes?

As Kevin mentioned this should be working as expected in the latest release of Premiere Pro CC 2014.2. You will no longer need to change to Software rendering.

Premiere Pro Events lists warnings, error messages, and other information you can use to identify and troubleshoot problems, particularly those associated with plug-ins and other components from third-party developers. An alert icon , , on the status bar notifies you of an error. Double-clicking the icon opens the Events panel, and clearing the associated item from the Events panel removes the icon from the status bar.
Do either of the following:
Double-click the alert icon in the status bar.Choose Window > Events.
Do any of the following:
To learn more about an item in the list, select it and click Details.To clear the events list, click Clear All.

Sorry for the delayed response I just stumbled across your post. I hope things are going smoother for you now and if they are please share how you corrected it.
First things that jump out are your running on very old software versions. I’d update to at least Mac 10.8.5 or higher. I’ll also update your Blackmagic driver to the latest version. They’ve made many fixes with audio sync related issues since the driver you listed.
