If you do a search here at the COW and on the web in general, you certainly come up with enough answers. Off the top of my head I can dimly recall at least 4 tutorials (2 for AE, one for Fusion, one for combustion*) that came out when that effect was all the rage.

You can’t. It’s as simple as that. Unless you can get a hold of a third-party plugin set called Expression Effects (which seems close to impossible), there is no way to get that functionality. There are some JSX scripts that can do something similar by creating keyframes for the mask shape, but that’s pretty much the extent of what’s doable in AE currently.

Film them as real footage and use them as mattes.

Yes and no. You can export the parts that are not going to be procesed further as DV, but areas where you want to add effects or transitions in Premiere should be lossless to avoid degeneration and artifacting.

No. Text presets often use features unique to text layers (as per your example a wiggly text selector for instance) and cannot be applied to standard footage items.

Check the following settings:
a) Audio sample rate in your After effects preferences.
b) Settings in your audio output device. AE does not play anything beyond 48 khz and it will only do so in stereo at the most. Turn off all surround and DSP settings, that your audio chipset may provide. If it uses a special driver, try to get rid of it or at least temporarily disable it and rely merely on Windows’ DirectSound and standard audio routines.
c) Limit the preview range by shortening the work area (the fat bar at the top of the timeline will do that for you). If it exceeds a certain length (depending on how much RAM you have) it will not play the audio (though AE builds the preview).

Yupp. At least the segments that were modified will be re-encoded and show more artifacts.
