All you will see if you connect a display to the Q4000 card, is the same thing you’ll see if you connect it to the second port of the GUI card. Just another desktop screen. You cannot see full video. You need the Decklink card for that.
Connecting a display to the Q4000 will reduce performance substantially, and is not advised. It’s dedicated to GPU tasks, and if you connect a display to it, it will also have to do display tasks, and that slows down the GPU processing.
What type of input does your display have? The Decklink has HDMI out, in addition to the SDI outs.

I assume you are asking about a power splitter because your 4870 card is already using both the Aux power connectors? I am not a Mac expert, but it seems no one recommends splitting power like this, as it may overload the Mac power supply. I think your better bet would be to replace the 4870 with something that only needs 1 Aux power connector, like a 5770 card.
The GUI display is the display that you see the Resolve GUI on, so it would be the computer monitor. It would connect to the card that is NOT the Q4000 card. The Q4000 card should have nothing connected to it.
Also, since you are running OSX 10.6.8, make sure you go to the Nvidia driver download site and download and install the latest Q4000 for Mac driver for 10.6.8, and download and install the 4.0.19 CUDA driver.

On the Browse page, in the Media Pool area, highlight the clip you want the Matte to be associated with.
Then, find your Matte clip so that it appears in the Clip Details area. Then right click it, and select Add as Matte.
If you look at the clip in the Media Pool, you’ll see it now has a green icon, and if you click that, it will expand and show the Matte clip attached.
The key is to have a clip highlighted in the media pool before selecting Add as Matte in the Clip Details area.

I’ve just done a test here. I have a system with a GT120 and GTX285. I made a project with a ProRes444 HD file, and did 5 nodes of various types of corrections. It works fine. I pulled the GT120 card. I do get the warning message now about the display GPU being used for processing. I load my ProRes444 project, and it works fine. Just to be sure, I made a new project with the GT120 card removed, and did primary, secondary, windowing and blur, and it all worked fine. The blur (5th node) did drop me out of real time playback, but that would be expected with a single card. But no noise artifacts as you describe and no other error messages.
I am running OSX 10.6.8 with the newer Nvidia 256.02.25 and CUDA 4.0.19 drivers. That might make a difference, but I doubt it. This system is also a 4,1 2009 Mac Pro. What type of Mac Pro do you have? If it’s a 2008 3,1 system, that might be an issue.
If you could send your Mac System Profile file in XML to davincihelp@blackmagic-design.com, we could take a look and see if anything looks odd.

Resolve does not require you to load any Wave software on your system. The Resolve app communicates with the Wave directly.
There is no ability to remap Wave controls under the Resolve app.
On the Blackmagic Design web site, in the Support area for Resolve Mac, under Support Notes, is a PDF doc that describes the Resolve Wave control mapping.
