The version I am using is 5.1.2600.0
Note that my issue is similar to yours only in the delay between internal and external monitors, but I did not have the complication of the NTSC thing oir FCP,so I have no idea if it is even a fix for you.
BTW: I am a PAL Lander.

I would suggest the video card plays a big part in PremPro 2 and may have even be documented. There is definitely the option to use GPU acclerared effects or two other options. A top end video card is advantageous in PremPro and many of the plugins are written for them. eg MAgic Bullets
Is your card on the approved/tested hardware list?
I am not sure what your original issue was but try using PremPro with playback settings as Compatible (not GPU Accelerated) using your current card.

Another little tip that maybe helpful.
I had an issue with the video on PremPro displaying with an approx 1 second delay at the external monitor. ie the video on the PremPro monitor and the external monitor were about at a second variance. (Irritating to say the least) – turned out to be 17 frames variance.
I fixed it by changing the msdv.sys file to the version used in Service PAck 1.
If you are still with me here…
This created a new litle problem. My dubbs to tape were exactly 17 frames out.
I solved this by simply deducting the 17 frames from the Tape Record In point and this works fine and reliably. eg my workbar in point less 17 frames.
Kind of a simple way to make the off set if you need any. (Just have to rember how much it is all the time)

That is probably correct because when it imports to ProTools it references all the clips from the PremPro timeline to 0000000 etc in the Protools timeline as a nominal start point.
I have found the OMF export lines up in Protools fine without even considering what the T/codes are.

Have you tried without your 150 frame preroll.
eg the 4 sec 12 frame offset is 152 frames difference (@30fps)
I use no pre roll on my DSR11
