1. Audio clips that are mono can only be placed on mono tracks, stereo on stereo tracks, etc. If you want to add audio tracks to get the right kind of track into the first track, it is easy enough to do so.
2 Yes, it happens to me sometimes as well. Click on the transition and change the setting to “center on cut” in the Effect Controls panel. It may be that you don’t have heads or tails trimmed properly, or it may just be stubborn.
3 Adding to the end of the sequence is a simple matter of moving the Current Time Indicator (CTI) to the end of the sequence before you insert or overlay. To add something to a different track, select the track before you insert or overlay.
4 There are a lot of tutorials available, but you might want to pick up a book (shop at the Dairy Store here at the COW to be a good citizen – top of this page has a link to “Store”). There are a few podcasts and tutorials here as well.

Premiere Pro should not need to conform every time it opens a project. Are you changing drive letters somehow?
Do you have the administrative rights on that PC?
Try deleting all of the media cache and preview folders. Then open the project and let it conform. It should not have to do so again unless it can not find the files.

Three frames is only one tenth of a second. i really don’t think it would look like anything other than a glitch in the playback.
However, you could easily use the razor to slice one frame away from the clip, take it up to the next track, expand it out to the three frame duration, then insert it back in. Set the CTI to go ahead 5 minutes and then do it again.
There is no automation for something like this. Would you mind explaining the purpose of a one tenth of a second freeze frame?
I suppose it might be possible to adjust time remapping to do the job and then possibly copy and paste the keyframes? I am not on my editing PC this week.

Sorry, I haven’t seen Grindhouse yet. My guess is that you might want to create your own overlay using the same methods. Any particle software ought to be able to do it, but Particle Illusion is the easiest to use.

No, there is no way to do that. But you might want to reconsider your work flow.
If you select portions of your clips in the Source monitor, then put them on the sequence, you won’t have that problem.
