Progressive download requires a HTTP (not FTP) server with MIME settings set for .mov files.

As an aside, Premiere lacks Sorenson Professional codec requiring twice the bitrate of that and Windows Media or H264 compression.

Saving titles created from one template in version 2 was pretty dumb, requiring renaming the title in the Project window.
CS3 changed that; adding “Name” to the bottom of the Title window. -
My workflow is similar. I export each sequence as Mpeg 2 DVD and import into Encore. Each sequence gets its own timeline and menu chapter. But my logic is only that is would be easier to make a last minute correction to one sequence rather than render the whole thing again.
Shooting weddings wears me out! -

How do your Project Settings (resolution) compare with the dimensions of the title?
Using anti-aliasing in PS can cause display issues on a TV. -
I have heard of issues caused by defraging a drive but suspect they are urban legends. I defrag prior to capturing or just after dumping a project.

P2 support began with the 3.1 (not 3.01) update for Premiere. If you have a trial version, you may not be able to update, but try.
I thought that transferring data from media cards to PC would be a Windows copy/paste function and not involve the NLE. -
The ideal workflow would involve playing the VHS tape to an analog/digital converter (or mini-DV camcorder) and capturing that via firewire into Premiere as Digital Video.

The process you used sometime does not work, so you might explore plan B.
When it does work, the result is recompression of already compressed video and a generational hit.
If you dont need more than simple cuts and want to avoid the degradation of recompression, the best bet I have found is VideoRedo Plus. It will allow you to slice and dice and then will stitch scenes together without recompressing. The I take that file into Encore to author. -
I have seen this using VOB files that where renamed to .mpg. What is the source of your 2 hour Mpeg file?
Editing Mpeg files is always dicey.
