It’s pretty common in the duplication/replication industry to require full payment prior to starting an order. We have the same policy for first time orders, and then allow business clients to request credit terms upon a credit check.
Most of our clients understand the policy and don’t mind, but to the few that are uncomfortable I suggest that they use a credit card for payment instead of a check. That way if we were to not deliver, they could dispute it with the card company and likely not have to pay. Most of our clients pay with card anyways.
Have your voiced your concerns to them honestly and respectfully? It might be worth shooting off a quick email that clearly voices your concerns and asks for an update on your project. It might not change anything, but at least it lets them know your concerns.

In Illustrator CS 4 (and I think 5) on Mac, the fonts are only previewed from the Character Menu and Character Panel, but NOT the control panel.
If that doesn’t take care of it, you might want to check that you have Font Preview enables in the Type Preference pane (ILLUSTRATOR > PREFERENCES > TYPE > FONT PREVIEW).

I love toast. It’s like a swiss army knife. But it can be quirky.
Sometimes I find reinstalling and trashing all the preference files can fix a lot of issues. Might be worth trying before you troubleshoot othe issues.

On the text box that you want to be the caption (ie. NOT wrap), you need to tell it to ignore text wrapping.
Select the text box and then go to the OBJECT menu and select TEXT FRAME OPTIONS. In the dialog box, check the box for IGNORE TEXT WRAP.
Should take care of it.

After you export the PDF from InDesign correctly, you need to open the PDF in Acrobat and set the default initial view for that file. This is what controls how the file will display when people open it. They can always change it to their preference, but you can at least pick how the magazine is FIRST displayed.
In Acrobat 9 Pro for Mac, you go to FILE > PROPERTIES and then click on the INITIAL VIEW tab in the dialog box. Set it to your liking and then save the file.

DVDs don’t use the line 21 method for closed captions. They are encoded in a specific caption file and added to the DVD in the authoring process. When the DVD player outputs the signal, it is converted to line 21 for the TV or display.
I am not aware of any way to add this after the fact, but then again, I’ve never tried.
