Loading a movie clip is the same as loading a background image, just choose the video file and it should load up (AVI on Windows, Quicktime on Mac, and you can load sequences of images on both).

I started writing a text version of how to disintegrate someone a little while ago, but it isn’t quite finished yet. There’s only a few steps missing from the logo one to make it viable for destroying humans.
-Shoot the actor in front of a green/blue screen, pull the key from the footage in an editing or compositing program, and then import that background footage into pI with an alpha channel (a solid actor in front of a transparent background).
-Rather than hiding the background image with a box like in the logo tutorial, use the Background Image controls to turn off the footage of the actor at a specific frame (you can only turn off the background on a certain frame if you have the 3.01 update. In 3.0, you needed to edit the clip of your actor in your editing program so that it ends on the frame that you want the actor to dissapear on).
-Use an area emitter rather than a line one, make it cover the area where the actor is on the frame you want him to start disintegrating. By using Get Color and Get Transparency from Layer, you’ll be creating a duplicate of your actor out of particles, and you can then adjust the Weight, Velocity, etc to destroy them as you see fit.

If things are getting shrunk down, it’s because the stage is covered up in some way by the other windows of the interface. The quick way to get around this is to hit Alt+3 before rendering, which will maximize the stage, or you can just drag the edges of the other windows so that your stage is big enough to fit a 720×480 area.
What codec are you using to output the final renders with? The codec you choose and its settings will determine how large the file size is in the end (though that doesn’t affect the length of the clip, just the quality)

Try running Reset Default Settings from the particleIllusion group under the Start menu. Usually if there’s any odd behaviour, running that and then starting pI again will get it going.

Strange. If you just add one frame of the sequence, does the program quit too? You may want to try to add them to the emitter but not the library (the button looks the same, it’s over to the right of the one that adds them to the library in the Properties window) to see if that also quits.

You can change the default length of the project from 120 frames to whatever you like in the Preferences.
If you have a compositor that has a tracker, that may be the best way to track the eyes. If not, you’ll have to manually match them up. When doing it manually I find it best first to move the emitter into place every 5 or 10 frames first, and then adding more inbetween keyframes as needed.

The 1200 mentioned are the ones available on the download page. If you download the library viewer ShowIEL, I think it has most of the libraries included in the zip.
The CD has all the libraries from 2002 and earlier reorganized into categories (fire, smoke, sparkles, etc)
