By having the tracks layered I can select the best take from the best point in time of the music and just turn that on and the other tracks off. Any suggestions?”
So I assume you have 4 takes, but each take is multiple angles?
It appears that Premiere will only properly multicam sequences if they are all in the same layer of video – whatever layer the first multicam of the sequence is, lowest taking precedence.
I would convert your video to 640×480, multicam all your cameras together (hard drive willing) and do a larger single multicam. Once it’s rough edited, you can reconnect your high res original footage.

But if I select Audio Gain on that clip and adjust the gain by say -20db, the Clip Mixer still shows my original -7db adjustment. I interpret this to mean that the Audio Gain command is “pre” adjusting the clip audio, and that adjustment is rippled forward into the Clip Mixer where it’s adjusted again by my -7db setting.I can get my head around this somewhat, but why would the Audio Gain adjustment be invisible?”
Yes, and as to why it’s invisible is that the Audio Gain setting is considered an interpretation setting (much like framerate can be interpreted for video), and is a pre-gain stage before it hits any effects or clip/timeline mixer.
As to why do it there? Some people like mixing near unity (0) and would rather see small adjustments on their mixer like +1db, -2db rather than deal with wild swings or boosts outside of the gain of the clip and timeline mixer.

Save it as a nested sequence, then you can load it into the source monitor by dragging it from the bin to the source monitor or using ctrl+o when it’s selected in the bin.

If you aren’t able to do ProRes then DNxHD (either MXF or MOV wrapped) should be fine https://avid.force.com/pkb/articles/en_US/download/Avid-Codecs-LE-2-3-9
As far as h.264, I would do whatever you consider suitable quality for YouTube or Vimeo. This will probably act as your submission screener. Again, I hesitate to spit out a dozen different formats without knowing submission specs – things can be all over the place, especially preferred sound mix and channel assignment (I hope you have at least a left-center-right mix for festivals).

Output DNxHD 220x or ProRes HQ 422 as your “master” formats. Generally each film festival will have delivery specs. If they require a DCP, I suggest paying the lab fee for them to make one, in which case you can deliver them either of the above to prep one for you.
DVD is, of course, DVD so it’s MPEG-2 and Premiere has output settings optimized for that.

export as an image sequence – if you say that the white frames are fully random, then you can see the white frames in Finder and re-export those portions until you get a clean render you can convert to a video file using Premiere or AME.
Sometimes getting the work done trumps figuring out whats happening when you have client deadlines.

So to summarize you have a video file or nested comp from AE that is one solid file?
Save an EDL from your original timeline (as long as the video is sanitized for EDL, in other words, one layer of video only) and then in Speed grade you should be able to open the EDL in the media browser and drag it onto the solid black from AE and it will cut it.
If your project is too messed up for that, you could use Speedgrade’s Scene Change Detection.

h.264 is a delivery format and CS4 doesn’t officially support optimized editing of it.
Look into some intermediate format workflows like ProRes on Mac or Quicktime wrapped Avid DNxHD https://avid.force.com/pkb/articles/en_US/download/Avid-Codecs-LE-2-3-9 for your export if you plan on re-editing.
