That is a known problem with Premiere Pro 2.0 and is fixed with CS3.

Camtasia Studio from Techsmith is perhaps the best tool around. They have a free 30 day trial as well.

Have you tried previewing the DVD in a DVD-software, rather than on TV? What about a different DVD player?
