In 2015.3, Premiere does not currently support the metadata from the Proxy clip. It will only read metadata from the Full Res clip.
AVCHD is a compressed format, so yes, it will reduce quality each time you render/export. With the Proxy feature, the encoded Proxy is an attached clip, so you can still switch (Toggle Proxies) to your Full Res clip that has no quality loss.

Possible workaround is to change the Position value in the Effect Controls panel. Twirl down the triangle for Motion to see the parameters. Under Position, click on the left value, keep the mouse down and move mouse to the right.

If you click Search a second time, then it should search for the next match across other locations and drives. If for some reason that still does not work, try manually going to DriveB and selecting the first clip for relink. If “Relink others automatically” option is checked in the main Link Media dialog (which it should be, by default), then it should automatically link the clips from that drive location.

If it is DV .mov, Premiere Pro should be able to natively read the file. Does it not work for you in Premiere Pro?
I just verified a few DV .mov files in Premiere Pro CC 2015 on my Windows 10 system with no QuickTime installed. They import and play without any issues.

This option is now under the burger/wing menu of the Timeline panel Sequence tab.

It’s possibly timecode related, as it appears that your clips are shifted. If you Offline the problem clips, then Link Media, try toggling “Align Timecode” and then Locate the clips. Does having Align Timecode checked or not checked make a difference?

Think I found the actual culprit. Go to File menu > Project Settings > General and uncheck “Display the project item name and label color for all instances” This will allow you to only change the color or name of the edit(s) you selected.
Now, there may be some update issues if you made some changes to edits and then check or uncheck this option.

I would also say that looks like a marker. If you double-click on it in the Source monitor, it should open the Marker dialog.
