What I did to recreate this in Premiere Pro was to animate the levels effect starting at normal at a dozen frames before the end of the clip to all white at the end. This blows out the whites first so I like it better than fading to a white clip.
I save the effect as a preset and mark it for “Anchor to Out Point”.
I do the same thing in reverse for the first part of a clip. Set it to normal at 12 frames in and back up to the beginning and make it all white. Save it set to “Anchor at In Point”.
You will be able to easily apply this for any clip in any project. Remember that you might want to slice the video just previous to the effect or just after before applying the preset, or you will have to render the entire clip and not just the area of interest.

I use the garbage matte by moving it to a new sequence, lightening it up with the levels effect, putting it over a black matte, and using the sequence as a track matte nested in the original sequence. Blur the clip with the garbage matte and it will make a soft edge.

It comes in very handy being able to put some clips and effects together and then use them as though they were only one clip. It groups them, makes it easy to transition to them, and various other advantages.

Import the 16:9 project into a 4:3 project. It will cut off the edges unless you decide to pan left or right a little to get the best part of the 16:9 frame into the 4:3 frame. Export from there.

It is probably being treated as a nested sequence. Can you drill into the bins and find the original in there somewhere?

And crashes apparently. You might want to consider that some of your problems are Matrox based.
