Hold the Shift and Ctrl keys down while launching Premiere, deletes Premiere’s Preference file.
Also try a new Project…the old Project file may be corrupt.

Not if you can hear other audio from the headphones.
Try this:
Edit>Preferences>Media and then click the Clean button. -
Using Premiere, audio can be output to your PC soundcard OR an attached DV device, your choice. The choice is made in Playback Settings.
Even though the setting may appear correct, reset it anyway. -
Increase your page file to 6 GB.
I have heard folks having success with CachemanXP, a memory management app that controls Premiere’s voracious memory appetite.In the future on large projects dont put all your eggs in one basket (project).

I would try reseting Premiere’s preferences first…hold the Shift and Ctrl keys down while starting Premiere.

Just installed the demo…you can also drag and drop the animation onto the text object after creating a new title.

No other settings are required. Are you using a 3rd party capture card? Are you in a DV project?

Too many is as bad as not enough…what is the first item?? turn it off, uninstall it or disable it.
