Also, if you decide to use the Behringer SRC2496, use the RCA digital output into the AES input of the ATEM. Basically, a cable with a RCA adapter on one end, and a BNC adapter on the other end.

I take that back. It looks like on the Behringer that AES is output via the XLR output. So you would need a cable with XLR on one end, and BNC on the other. -
While I agree with you on the VU meters in capture/ingest, saying you need third-party software isn’t actually accurate. You just need to use the software that came with your capture hardware, which I wouldn’t call third-party, PPro is closer to third-party.

Yep, the VU meters don’t work during capture/ingest in CS5.5 or CS6, but work with Black Magic’s Media Express (using a BMD card). Unfortunately, when using Media Express, you don’t get all of the codec options that you do with PPro.

To the OP, I feel your pain, as I learned this the hard way.
I was hired to shoot, direct, and edit a pilot of a restaurant/traveling show. They hired their own audio technician, which used a boom (above and below the talent), but used no dampening material.
The audio turned out to be hideous, sounding quite like your sample. The worst was during shots in the kitchen, nearly (and I would say) unusable.
I’ve always personally used lavs, and never have had any major issues.

Just update to the newest firmware and drivers that are available on that page.

Try straight from the HDMI to a monitor and see if it works. If it does, than you’re good to go.
Make sure to set preview/playback to the HDMI monitor once you restart Premiere, obviously after connecting the monitor first with Premiere closed.
The way I do it is connect the monitor, set it as a second display (not cloned), open Premiere, and set the playback device as the second display.

Best thing to do is simply test it. A display port to HDMI adapter only costs ~$5, hook it up to a cheap monitor with HDMI inputs. If that works, then the HDMI-to-SDI converter should work.

Are you using both of the outputs of the Quadro? If not, you could possibly use the display port, get an hdmi adapter for it, and then a HDMI-to-SDI converter from BMD.
