i have seen this before with my IO. i think it was a corrupted sequence — copying to a new one fixed it.

it may have something to do with splitting the audio and video to separate files on capture. developing….

i’m seeing the exact same problem with the Aja IO on clips as short as 5 minutes. what type of storage are you capturing to? i seem to only see this when capturing to the XSAN, not to a HUGE SCSI array.
