This is the only correct approach, but you don’t exactly have the right understanding yet.
It’s not possible to make a video file into a menu – you must have a PSD file with the correct layer prefixes to have a menu in Encore. So what you do is create a PSD that has ONLY the button layer sets with highlights within it, and then add the video to the menu background.
You then set the Loop Point property to the timecode where you want the buttons to be active.
That’s pretty much it.

Sounds like you may be missing a codec.
Encore version 1 was released in 2003, several years before Vista. Last I knew, Encore 1.0 only supported Windows XP.

Encore has a different burning engine than Nero. If you prefer Nero’s, use it.

Not with Encore alone. You can add the PDF in the ROM folder, and then provide the viewer with instructions on how to access it.

Add chapter points to the timeline, and then link buttons on the menu to the chapter points.

You exported as MPEG-2 from Premiere Pro 2, and then transcoded again in Encore?
That might cause the flicker, and would certainly result in a loss of video quality.

Not in Encore.
You will have to convert the video to NTSC, and then create a new NTSC project in Encore and rebuild the project from the ground up.

You could make two menus – one in its entirety, and the other with just what you want.

That’s not really the best way to tackle this.
Instead, use the video clip as a background for your menu. You’ll probably want to pad out 10 or 15 seconds in AE for the still part of the menu. Export this as QT or AVI, then import it into Encore and set the menu background to the video file.
Then set the menu Loop Point property to the timecode when you want the button to become active.
