In general, you can just copy the data from OMFI Mediafiles directory onto a new computer. Delete the media databases and go – copy the bins as well and all the media should relink okay (although things like precomputes often become ‘orphans’ in the process, but that shouldn’t be a problem).
HOWEVER, it seems Windows XP can have serious issues with file permissions sometimes, and Avid can create file permissions which no user (including Administrator) can change, which can result is unusable media files, which can’t be renamed, moved or deleted.
I don’t quite know under what circumstances this does happen, but it it certainly can happen. To work around it in the past, I moved media in as few goes as possible, onto seperate drives. That way if I got media with messed up permissions (it only happens with the move, once it’s there and working it will stay working) I could reformat the drive and try again.

DV25 411 (or 420 for PAL) is native DV resolution – that’s what’s stored on the tape. So for capturing DV-originated footage, DV25 over firewire is good.
But 1:1 is better if you are adding graphics or effects.

A narrow angle lens (200mm) will give you no distortion of vertical lines and such. But pretty much anything beyond 35-50mm should be okay. Anything wider than that and you will start to get minor ‘fisheye’ distortion.

We normally use TIF/TGA sequences.
Otherwise, you can get the Avid AVR77 codec from the Avid site, which can be helpful.

I’ve finished episodes less than 24 hours before broadcast in the past. After a 22 hour shift to make all the executive producer’s changes that the Producer should have seen coming.

The shape of the 6-pin plugs (with the little arrow) is such that it is still possible to plug it in around the wrong way (rotated 180degrees). It usually is a little difficult, but it will go if you force it.
I don’t know about avoiding it.

When using 6-pin Firewire connectors, it is possible to plug them in around the wrong way, which puts 30v DC down one of the RX/TX pairs. This can (and does) fry firewire chips, on cards and firewire devices. I have seen it stuff a number of drives and even a deck once or twice.
I don’t know for sure if that’s what has happened to you, but it could be. You should be able to get a service centre to check your deck.
