Without actually seeing your setup, it sounds like something to do with the way you have your tabs set in your TOC style. Did you set a specific tab placement or just leave it as the defaults?
If it’s the latter, I’ll bet the ones that aren’t working correctly are the shorter chapter names.d.

I agree, it’s a major oversight.
I’ve got an old script for duplicating documents over at https://wp.me/p1Foaz-a3. Does what you did — create a new document then copy/paste the content across.
Used to use it a lot in CS2 then CS4 until I realised I could “Move Pages” instead.
Should convert to newer versions of the CS OK.

Did you ever find a way to do this?I looked into it a little, but it seems that superscript is a document-wide preference.
I’d love to know if you managed to crack it.

InDesign seems to have problems resolving page size issues in at least two scenarios that I know of :- A document starts life at one size and is later changed to a different size; and
- The page tool is clicked on a page, irrespective of whether or not the page size is actually changed.
As far as you can tell, the pages are all the same size. But as far as InDesign is concerned, something is amiss.
Two things to try :
- Create a new document at 8.5×11 (it needs only one page). Go to your original document, select all the pages in your Pages panel, then use the “Move Pages” command from the Pages panel dropdown menu to move all your pages from the original file into the new file (which you select under the ‘Move to’ dropdown).
- If that doesn’t work, try creating new master pages to replace your previous ones and then apply those throughout the document.
I found that, for my particular issue, I had to do both these things to get a satisfactory result.

I guess you’ve already worked these other two out by now.
I’m just posting an answer in case others have the same question.#1 : Check Arrange under your Window menu. In CS6 you can “Float all in windows” then “Tile”. It will be something similar in CS5.5 — have a play.
#2 : Right in the top left corner of your window you have a grid of nine squares. The default (black) square is top left but you can click on whichever one you want.

I’ve looked at this again in Acrobat Pro and my initial suggestion still stands.
Here’s the difference between the two files in Acrobat’s page display :
Export your file as single pages (rather than spreads) then go to File > Properties > Initial View and set it like this :
You should find that it will behave the same as mageg.pdf

I’ve had a quick look in my old version of Acrobat Reader and both files behave the same (I thought you were talking about a page turn thing with the mag but just looks like a standard PDF).
Mageg is set up to display two-up (that is, it is exported as single pages and displayed as spreads). Categ displays as single pages because you’ve exported it as spreads. The buttons in Categ work fine.
So, I’m not sure what difference you are seeing. I’ll have another look at them tonight on my other (newer) machine.
Have you tried playing with the the View > Page Display settings yet?

Are you saying that you can see a page curl like in a flip book as you turn a page?

Maybe try exporting the InDesign file as a PDF (or jpg).
Place that in Illustrator (File > Place) — it should come in at 100%.d.
