On brush tip shape, change the spacing to 0%.

I’m not sure if I understood your issue correctly, but it seems that you’re in full screen mode, if that’s the case all you need to do is hit F to toggle between standard, full screen with menu and full screen.
If that’s not the case, then go to window and see if Application frame at the bottom is checked, if it is, uncheck it.
Regarding the masks, it really comes down to how you move the mouse/pen across the screen, maybe you are releasing the cursor too far from the image and you are getting an almost grey gradient which causes your image to fade evenly, try releasing the mouse before so you get a more contrasted gradient.

That’s a context sensitive menu that normally is activated by clicking the control key on your board, at least on mac. Could it be that your ctrl key is stuck? If not try reinstalling photoshop again. When I upgraded to CS5 the one piece I had to reinstall was Photoshop, when I got the upgrade in my office the same happened, chances are that will fix your issue.

It might be that you have some transparent object left by accident outside the artboard, when you choose to export to .png (or .jpg, .tif, etc, for that matter) and do not click the use artboards option Illustrator will export everything on your working area, not only the artboards, your issue seems to fit that description.
Check for stray points, empty text frames, or any object that you might created, willingly or not and that was left in there, or export the artboard alone.

Change the anti aliasing to smooth, also check that you have the correct font file installed, you might be missing the postscript or the screen file.

Unfortunately you will have to trace the logo by hand, Livetrace is not that accurate, specially for tracing logos and create outlines is only used when converting type to objects.
Try placing the file that you’re basing your trace of as a template layer (file>place click template at the bottom of the dialog box) and then create a new layer on top, as you did already. The white mask you’re referring to is appearing because you’re using a path with a black stroke and a white fill, so as the shape progresses some areas of your image begin to get covered, you can avoid this by either working in outline mode (cmd Y) or by removing the fill from the shape you’re tracing (in the tools palette, at the bottom, or in the color panel).

Try saving to a different version of PDF, when in the options window pick a different version in the compatibility drop out menu.
