If the laser diode in an older DVD player begins to weaken, its light output will be less, and the player will have trouble reading any burnable DVD. The player probably will continue to play replicated DVDs correctly because these reflect much more light back to the sensor.
If seen laser diodes in DVD players go bad in as little as six months of use.

The short answer is that your menu is in the VMG (video manager) on your DVD, and that, by DVD Spec, is hard-wired to the “Title” or “Top Menu” button on the player. To access the menu via the Menu key, the menu must be a title set menu, which is hard-wired to the “Menu” button on the player.
I’m not sure whether you are using Scenarist, iDVD or something in between, so I can’t really advise further.

We don’t normally use this kind of workflow, but I believe the solution lies in building two sets of dual mono tracks in Avid, exporting picture and the first “stereo” pair to QT, then export a second QT audio-only with the second pair. You should be able to import those into DVDitPro without any trouble.

Not without software like Mac Caption. Final Cut Pro 7 does not have any caption authoring capability whatsoever.

I would export a Quicktime reference file from Avid and do the DVD encoding in Sorenson Squeeze. All NTSC DVD is 720×480. Your encoding software/hardware will downconvert to that size and make the image anamorphic, so be sure to encode as 16×9.
For better quality, you can export an uncompressed Quicktime file and feed that to any of a number of high-end encoders.

You need to buy a Firewire 800 to mini 4-pin Firewire 400 cable. The Apple stores sell them, but you can get one cheaper at any of the internet sites that sell computer cables. I had the same problem when I needed to use my DSR-11 with a new Nehalem Intel Mac Pro.

A DVD disc must be either NTSC or PAL with 100% of the assets being in the chosen project standard. That means, if you are authoring in PAL, anything in NTSC or HD must be converted to PAL.

The main install disc has a folder with Extras, but it has things like academy leaders for Cinema Tools. I don’t recall seeing any other test patterns.

No. You need to start at 1024×576 square pixels and resize down to 720×576.
