I think that is your problem. 4K (H.264/5) does not edit easily, might want to try proxies.

Do not use the mask on the Crop effect. It can cause strange behaviors.
Any other effect will be fine: i usually use Opacity.

Red Giant has LUT buddy which you can download from their site and use in CS6.

There was a known bug with renaming audio tracks, but cannot remember which version.
Might want to update.
