Dont use the preview files, use Max Render Quality.

Use the Effect Controls: you can scrub or either fill in numbers.

It seems that way, but Premiere makes a file of the extracted audio which is located in the project files on your hard drive. Open that file in Soundbooth and you can edit again.

Probably not possible witout the black frame, whit such an odd resolution.

Edit in HD and export to the required format.
I would not consider 4:3. You will loose a lot in the sides. I think letterbox is a better option.
