I wanted a screendump to see for myself, codec installed does not mean it will work in Premiere. But wild guess.
If its AC3 copy the file ad2ac3dec.dll from
the Encore CS3 directory and paste it into the PPro CS3 root directory
Re-import the files.

If you mean the Clip effect that was last used in Ppro2.
Crop or size down the video and put a color matte under the video layer.
Nest both layers and you can rotate or move it around in the frame.

This has been reported the Adobe forum once (Mac also) but unfortunately never been solved.
I think re-install is the best option or check the firewire.

That is correct. No preview in the capture window only on the camera lcd screen.

I would ignore the first line as we are in the HD era.
The article gives great info on how to make good titles, most of his suggestions still apply.
Your problem is probably a interlaced artifact caused by the font you used.
It has very thin horizontal lines which might be just as wide as a scan line.

Most laptops are not suited for CS5.
Recommended is a i7 9xxx 12 gig of ram and a dedicated graphicscard.

In fact its very simple. Upscaled footage looks awfull. Its very noticable if its mixed with high resolution footage.
Its better to downscale.
Your project settings should read 720p.
