Edit > Preferences > Disk Cache… make sure they’re on your D drive, or better yet an internal drive that has space but isn’t the same as your footage.

1. Add an adjustment layer with gamma, contrast, or curves effect, adjust to taste, then use the pen or shape tool to create a mask around the piano player’s face. If the camera moves, you can motion track the piano player and add the motion tracking data to the adjustment layer so it moves in concert.
2. Use dynamic link to bring AE stuff back into Premiere, no need to render a full file for the trip. Premiere will make you render the timeline to continue editing in real time though. Sould be under file > Import and you can import an after effects project and select the comp you want to use.

This is a guess, but do you have the little sun-like icon checked for the text layer? That switch rasterizes vector layers… probably has some effect on how motion blur is calculated.

That’s fundamentally different than the Video Copilot tutorial because you aren’t breaking one layer down to reveal a background plate. You’re working with 3-4 layers (the flaking skin, the arm, and the body. The bg being the fourth layer depending on what’s going on). I would also think you’d have to wrap these particles in 3d. You could try something in a 3d suite like 3DSM or Maya, but if you haven’t shot yet you may want to consider doing it in a practical manner with layers of liquid latex.

Are you playing the export in Quicktime or roundtripping it and playing it back in Premiere?

If you’re using paper and vinyl indoors then two c-stand with sandbags can do the trick.
If you’re using it outdoors or plan on using muslin quite often, I might recommend 2 or 3 stage combo stands with a piece of speed rail… bullet proof.
I’m sure there are purpose built backdrop stands that are quite nice, but generic grip equipment tends to flexible for many uses.

Autoupdate always seems a little weird. When you save in After Effects it should push the update to Premiere (Premiere will bog down for a second or two as it does this) but I’ve also had times where it won’t: you have to clear your sequence render files in Premiere and then restart Premiere. Kind of a pain in the neck if it misbehaves.

What version of Premiere and After Effects? If it’s newer (I think CS4 introduced this) then use dynamic link.
Put each program side by side and literally take the comp you want to use from AE’s project bin and drag and drop it to Premiere’s project bin. There are more formals ways to do this using File > Import in Premiere… drag & drop is faster to explain.
In Premiere, drag the comp and drop it onto the “New Sequence” icon, this will create a sequence that conforms to your footage (frame size, frame rate, pixel aspect ratio, etc). You aren’t limited to sequence presets in Premiere, in fact you can edit them or create new ones to meet your needs.

Check your pixel aspect ratio on both your comp and export settings. Seeing 720 X 480 makes me think you’re using NTSC with a pixel aspect ratio of .91 (slightly rectangular). You need to be setting this to square (may be listed as 1.0 or 1:1).
