The switch to software only will cause your exports to take a long time vs. taking advantage of the CUDA GPU engine. Are your Nvidia CUDA drivers up to date? You can check this in the Nvidia panel under your system preference.
Exporting an XML from a future version of Premiere will allow you to import it into a previous version of Premiere. Not sure if you are comfortable doing that, but it is an option if you get fed up with the crashes and want to downgrade to a previous version of Premiere that wasn’t crashing as much.
You can submit your crash logs to Adobe as they will want to know about it vs. posting them here on the COW. What operating system are you running? Yosemite, El Capitan, Sierra? You can find this from the About This Mac menu.

If you copy and paste text into the titler in the latest version it will crash premiere. This is a known bug that Adobe is fixing for the next release. You need to provide a bunch more info if you expect someone to help you trouble shoot. System details, Codec of media you are using, sequence settings etc. More info the better.

Just remember to export a XML of your latest project so you can open up your project in the previous version of the software.

They are there in the new version. Are you searching for “window” vs. “panel”?
Also check your Window Menu and see if you see the actual Shift+Number combo next to the panel you expect your shortcut to go to.
Maybe your shortcuts are set to a custom set that did not translate when you upgraded. Can you try setting the keyboard short cuts to Default and try the Shift+1,2,3,4 combos to see if it matches your needs.
You can also try resetting your prefs when launching Premiere… “To restore default preference settings, hold down Alt (Windows) or Option (Mac OS) while the application is starting.”

Dan, you need to upgrade your CC2015 version of Premiere to the new CC2017 version. You will then see the ingest settings.
Go to your Creative Cloud desktop Application and click on Apps and you should see an update available for Premiere CC2017 version.
If you want to keep your current 2015 version to run along side CC2017, make sure you check the Advanced Options, uncheck the box “remove old versions.”

There is a whole new keyboard shortcuts interface. Just go through it and assign the short cuts you want for your panels. Default keyboard layout shows CTRL+Shift+, or CTRL+Shift_. will cycle the panels.

If you are on a mac, there are drivers for it on this page: https://www.aja.com/products/u-tap-sdi#support under the software menu.
If you have older Express drivers, it might conflict with the new U-TAP SDI. You can always call AJA support and chat with them.

It’s a know issue with the MP GPU’s… not only with the D700’s, but also the D500’s. Get Apple to replace graphics card A & B on your clients MP under their recall program.
FCPX will not show the same render glitches or lines. Watch the Activity monitor CPU menus peak out when Premiere or AME are exporting, then run the same test with FCPX. FCPX does not tax the CPU’s / GPU’s as bad and over heat the MP introducing the glitches. I’ve even had AME exporting, loaded up Compressor and exported at the same time. You’d think taxing the MP with both programs exporting it would fail on both. Compressor or FCPX don’t show the lines and glitches, but AME will. It’s not always consistent when you reboot to cool things down, but a lot of the times during my testing, Premiere & AME will fail on 4K exports where FCPX or Compressor will not.
Software only as you stated seems to be the only saving grace to get something exported out of Premiere or AME with a MP with these issues.

Try inserting a slug of tone audio and mute it under any gaps in your audio tracks. This should stop the ghosting audio.
