I don’t have a solution so take this with a grain of salt: have you tried running that HDMI output to a regular TV and see if it shows?
DVI isn’t quite like HDMI – it attempts to “talk” to the other end, get information about what that other end can accept, and adjust the parameters to match it. It’s entirely possible that the DVI port and the Intensity Pro HDMI input don’t quite understand each other.
What may work is running the output through any sort of a pass-through system like a TV or an amp with HDMI pass-through (output) that you could try connecting to the Intensity input.

One source told me this deal requires to own a point product, yet Videoguys’ page doesn’t seem to indicate it.

Another thing is that the card must support CUDA, be fast enough for GPU accel (GTX285 or faster), and either be supported by Adobe for it, or you would need to a do a little “hack” to enable it for GPU accel.
I.e. not every NVidia card will work.

Apparently you can upgrade to CS5.5 Production Premium all the way from Premiere Pro or just “Premiere”, for $1099. If “Premiere” means any version of Premiere including v.1.0 from 1991 then it’s a pretty sweet deal, as those should be available for next to nothing – I might even have one or two copies lying around.
As Ann Bens mentioned, make sure that serial number is valid and not used already for an upgrade – otherwise Adobe won’t let you use it.
I haven’t been able to find a competitive upgrade that would let you cross-grade directly from FCP for less money than the full version – I’ll keep looking though.

Jan mentioned that “the card has to work for CS5 as well” – I assume for GPU acceleration – which leaves pretty much one choice – Quadro 4000 Mac – regardless of benchmarks. It’s not a bad card. A Mac version of GeForce GTX570 or 580 would have been better (for CS5 or any other purposes) – as they are faster than Q4K in nearly every respect except OpenGL performance (which isn’t an issue for CUDA or OpenCL) – but alas, no such thing. So Quadro 4000 is basically the only reasonable choice for a Mac-compatible card that has GPU acceleration in CS5.x. (It’s only compatible with 10.6.x and up though – not 10.5.)

If it’s Windows 7 or Vista 64-bit – it will run but expect long transcoding, rendering and encoding times and possibly other issues. E.g. after you import the AVCHD or H264 HD video from the camera, it’s highly unlikely it will play right away – you’d have to wait till it’s transcoded.
If it’s anything other than 64-bit Win7 or Vista – no; at least not Pr or AE CS5.x; you have to use CS4.
If there is any budget to upgrade this system a bit, it’s (1) an extra internal hard drive (or a USB 3.0 card with a USB 3.0 external 7200rpm HDD), (2) a faster graphics card, (3) another 4GB RAM.

I see what you mean; some video I/O cards can indeed MIRROR main user interface display output to a video monitor or be used as a 2nd UI display monitor. I haven’t heard of a video I/O card that also allows to drive a UI display monitor in a headless (graphics-card-less) system. (The exception is of course a card that combines graphics and video I/O – like Matrox Parhelia – I think.)
