The last time I did ENT surgery, we were able to get great video from the scope. The monitor had BNC outs, which we hooked up a deck and recorded along with 2 cameras.

I would think that the docs would be wearing masks, so you wouldn’t really want the mic near their mouth anyway.
Check out some theatre techniques. They often place mics in hair, around the collar line, etc…
One place I’ve never tried, but just thought it might work out great – the overhead light. If it wouldn’t cause interference, the light usually is shaped like a parabola, so it would make a great collector for sound. On the other hand, it would also pickup suction, drilling, cauterizing very well too. So you would need to instruct the docs to not talk during those parts.

However, I would be more prone to do this in a layout program such as InDesign. Here you will have all the controls that you need to layout the entire DVD jacket. There are templates available from various resources. This way you can then create a pdf for the actual printing. That pdf will retain all of the vectors you create, rather than a jpg or something which will convert it all to raster. (Raster images may not be to your liking, especially text which might become stair-stepped.)

So how much faster is the x.264 than the H.264?
I tried a really quick comparison with an older version of the x.264 for a streaming application. The video quality was much better, but the CPU overhead was outrageous. But it’s probably not a fair comparison, as I am using a G4 for the streaming. With Apple’s H.264, CPU usage is about 80-90%. I couldn’t get a solid stream with x.264, so I moved over to a dual chip 2GHz G5. CPU usage there was in the 90’s for both CPUs! Again this is using an older version of the codec (as it is compatible with the G series processors).
This is 320 x 240 video (incoming video is DV on firewire) with 16kHz audio.

You state WHAT is will be displayed on (a 42in HDTV), but you don’t say HOW.

…and just as often you find people who aren’t aware of the quality difference. (YouTube offers “HD” files now, you know?)
Just as often you find people here with no clue about the software they are using. They get a project dumped on them by higher ups or someone downloads an illegal version to “play with it”, etc…..
This is the basics forum, so the basics should get covered. Plus a basic internet search would have revealed quite a few plug-ins, stand alone software and hosted sites which would perform this basic operation.
I may not have posted the best and most comprehensive solution, but I do try to dissuade those from performing illegal activities. If I found my footage being used in any way, I will prosecute to the furthest extent the law allows. So I’m not the most eager to show how to steal other people’s hard earned work.
