Shake4 works fine with 10.3.9 – it was the early version of QT7 that caused the problems such as 10bit material not FileIn correctly.

Have you updated to the latest version of quicktime?
Can you see any other ouput from an app through the card eg FCP?
Have you hit the video preview button in Shake?
Have you set the number of monitors correctly in globals so that it forces the preview to the card?
