I had a similar issue awhile back with a video file.
Seems like Premiere mistakenly believes its the same file as your first file. Might have to do with XMP metadata. Do you have that turned on?
Seems like some serious under the hood improvements are coming in the 2019 update announced at NAB. Hopefully, will lead to more advances in the toolset.
Yes, you will lose quality that way. You are blowing up the image larger than its raster size. You don’t reclaim that resolution by putting the result in a 1080 sequence.

Yes. Any rotation or rescaling of Video degrades it. Don’t know about Premiere v5, but current versions do a very good job and better than some other NLEs.
Put a Transform effect on the adjustment layer to adjust motion.
