See if you can find a frame or two from pre-roll or outtakes that has a flat background. Ideally, it would be a gray card, but you could get close with just a plain wall somewhere. To get a starting point, invert that and blur it slightly.
It will, at best, be an approximation. Be aware that it is only going to work at that particular focal length and you’ll need to shrink or enlarge it depending on the zoom.

If you are looking to hire someone to do the work, just ask here and I’m sure you’ll get many responses. Many of the folks on this forum are fantastic After Effects artists who can make almost any template you desire. However, that sort of talent comes at a price. Since motion-graphics is likely their full-time job, I doubt that you’ll find someone to do it for free.
If you need something less pricey, you’ll need to learn more about AE and build it from scratch, or shop one of the many template-for-sale sites. (You’ll still need a basic understanding of AE to know where to put the various elements and titles.) My guess is that you will find this exact motion sequence or something very similar. Envato, Videohive and Rocketstock come to mind and have many action sequences in the $20-$30 range..

Depends on which effects you mean. The 2-tone effect could probably be done with de-saturate and high contrast. However, the overall sequence looks like an ordinary animation, maybe even hand-drawn.. Sometimes, designers of unique title sequences will consider them ‘trade secrets,’ making it difficult to find out how it was actually done.
Google credits the opening titles to Zenon Kohler, so you might want to start a further search there.

You might want to try keying on the luminance of the overexposed sky. Then replace it with footage of a ‘prettier’ sky. You could probably even do that in Premiere.

If I understand your question correctly, you won’t need any plug-ins.
Use the >Transform >Scale control to fake the zoom-in/out. Obviously, the higher-res the original footage, the more you can zoom in. Use the Position control if you’d like to simulate the camera ‘hunting’ for the focus spot. I’d then use >Blur & Sharpen > Camera Lens Blur to fake the focus effect.
Should be able to set this up in 30-seconds or less. Tweak keyframes as needed.

The HBO miniseries Band of Brothers (about 2001) used an effect like you mention in it’s title sequence. It was frequently discussed here on the COW and other places.

I don’t think Time Remapping works as an adjustment layer.
If there are numerous layers in your existing comp, you could pre-comp your current composition and Time Remap the pre-comp.

Does it happen if you render starting at frame 26? I can speculate that there could be a corrupted frame causing some kind of problem.
Does it happen if you replace the source footage with something else?
Does it happen if you turn off ALL effects? You could then turn them back on one or two at a time to isolate the problem. I once ran into a problem caused by a demo 3rd party plug-in. The demo would render a limited amount of frames only . It was a borrowed machine and I was not aware that the plug-in was a demo.
One possible workaround is to render as an image-sequence. Frame 1 to 24, then 25 to whatever. Then render the image-sequence to whatever output you need.

I have had pretty good luck using Detail Preserving Upscale effect that comes with AE. It is a little hard to find because it is in the Effects > Distort menu. I have even had some old VHS upscaled to HD come out pretty good.
Somewhat like you stated, it’ll never look great. But depending on the source material, I have occasionally been amazed.

I have found that renders that fail at the exact same point are usually related to memory issues. Often times it is insufficient space on the destination drive.
