We usually keep our production PC disconnected from the internet.
However, I also find skype to be a bit un-friendly when using video apps. Either skype won’t find a connected camera, or the other app will cease to function. Quit skype, relaunch the other app, and things seem fine.

I’m have an older version, so not sure about CS6.
See if the PSD will work without the filter. It may be that the PS filter you are using is not available in AE. Some 3rd party PS filters seem to be purposely limited (so that you have to buy the same filter for AE?)
Also, I have used several render options in PS and can export QT, avi, mpeg, dv, etc. PS also has a submenu for the different settings.
For some reason, your Photoshop is not seeing these codecs on your system.

Let me be the first to suggest: Could you post 30-40 minutes of the footage? uhm…just so we could understand better?
(OK, got that out of my system.)
You may have already tried this, is there a way to key on the offending strap? That might at least make your work a bit simpler.

Yes, you can work with multiple layers in AE. Although, I’m not sure I completely understand your question.
A composition can have numerous layers. I would put the pattern in a comp, then another layer on top with the dots. Lower the opacity on the dots so you can see the pattern. Animate, then change the opacity back to 100%. If the dots move independantly, put them on separate layers.

As far as I can tell, there is no tool (yet) to truly fix out-of-focus images.
There are several plug-ins available claiming to fix the problem, but they mostly provide a combination of several sharpening techniques. What they do, they do very well and with some out-of-focus images, they can work wonders. Nik and Topaz come to mind, but I haven’t used the trials in while, so it may be other vendors. Pretty sure they are Photoshop plugs (only), so you’d have to pass your video through PS.
I took your image and tweaked the shadow-highlights, then some unsharp mask with a low amount (20%) and a high radius (7 pix) to get a little improvement.
One problem you will have with many video sources (particularly SD) is the large amount of edge-enhancement added by most video cameras. Any significant amount of sharpening exaggerates the edge-enhancement, making the image look “cartoon-y.”
If you find something to make big changes in out-of-focus materials, I’d love to hear about it.
