if Browser gives you a ‘failed to import the clip’ message the file itself may have been damaged or corrupted (though this is rare, and I’m a little skeptical based on the other issues you’ve had). Is this the error you get when importing an .mp4 to a folder on your system?
Do you have the correct AMA plugin installed for your version of Media Composer? And Transfer v 2.13? If you copy just the .mp4s to a folder and try adding that folder as a source in Transfer does that work? Note you should be using Transfer v 2.13; older versions do not support ‘orphaned’ .mp4 files).

yes, that sounds right, PPro, Vegas and others will often leave temp and cache files in any directories they touch. let me know if you still have problems after cleaning up BPAV..

Hard to say without seeing the file/folder structure on the card but have you moved/renamed/modified anything in the BPAV folder? XDCAM Transfer and Avid MC want to see Top Folder (eg ‘Card 1’)/BPAV/CLPR/TAKR . You should also see meduiiapro.xml on the root of BPAV. This is the ‘index’ for all intents and purposes. If these are not intact they will not show the clips.
It may be easiest/fastest to rebuild the BPAV by using XDCAM Browser (v2) to manually import the .mp4s to another folder. This reconstructs all the metadata and will result in a properly structured card archive. I have this documented here somewhere; do a search on Rebuild BPAV or something similar..
Also make sure everything is up to date– Browser v 2.0, Transfer 2.13.
Media Composer req’s AMA for EX; I’m pretty sure the mp4s cannot be manually imported. The above apps and also the AMA plugins are at https://www.sony.com/xdcam-downloads

You can rename the card in Finder (on the desktop, from Get Info etc) just as you would any other removable storage. There are no restrictions on this. Do a Get Info (CMD+I) and make sure there are no permissions issues. Also make sure the write protect tab isn’t on (I’m sure you’ve done this already but just in case..)
Make sure both the latest SxS and UDF drivers are installed and that they were installed in the correct order (Device Driver, then UDF)

It’s just a setting, you can view in low res Proxy (better for slower computers) or full res HD/SD. Change in Preferences–>PLAY–> MXF Playback Quality

XDCAM Transfer does not support SD, need to use the Log and Transfer extension or XDCAM Browser, these will convert to .mov. JFYI the .avi files can be brought into FCP but you’ll get a ‘media not optimized’ message and some performance issues (no destructive media management on non-mov clips, for example)

Are you planning on shooting in UDF or FAT? File naming is handled differently in each mode. in FAT the choices are Title/Plan; in UDF they are Cxxx (the standard generic XDCAM optical file names), Title or Plan. In either case the numbers are never random and advance based on what was set or a generic start # based on the serial # of the camcorder with an advancing suffix.

The numbering will continue from what you set, even if you reformat. The point is for you to be able to keep recording the file names you need across multiple cards or on the same card after offloading it.
If you use planing metadata you’d set Clip Naming to ‘PLAN’ and the camera would then automatically pull the custom file names from the planning metadata XML file.
Some manufacturers recommend formatting before every use but it isn’t really necessary with SxS unless you are getting constant ‘Restore’ messages or other indications that the file system needs to be reset. From a memory-health point of view the controller on the card is managing how/where data is written and always optimizing the writes to get the max lifespan. In most use cases a ‘Delete All’ will suffice.

You set this up in Menu–>Operation–>Clip. The numbers should not repeat, even if you format the card, until you manually change this setting.
