Sounds like you have installed QuickTime 7.5.5, which breaks the Media 100 codec. Either downgrade to QuickTime 7.5 (instructions can be found on this site), or wait for QuickTime 7.6 and keep your fingers crossed that Apple fixes this problem.

You need to upgrade to the Aja Kona V6.01 drivers. It will, as long as you do not upgrade to QuickTime 7.5.5. The new Apple ProApps update with FCP 6.0.5 does require QuickTime 7.5.5, though; if it is okay for you to stay on FCP 6.0.4 you will be fine. But the ProApps update will not get displayed in the Software Update anyway if you don´t install QT7.5.5. -

As far as I can see this only installs on the newer MacBook (Pro) systems with DisplayPort. I do not get it on our production systems, so I guess it does not contain a fix for our problem.

Yes, a very simple thing: FireWire HDV support was introduced in V12.5, and as you wrote your version of Producer is 12.0.2. This means you need to upgrade to use HDV over FireWire.

If you have RED already, you can do the same in Media 100 i using your copy of RED. Create a Composition Clip in the Media 100 timeline, and do the same as you are doing in the RED standalone version. Locate your source media file from the RED file browser, and scale it to your needs. Then render inside of Media 100.

You want to capture HDV from your Camera via FireWire? Then you should make sure that you have set your Video Input (In Preferences) to HD FireWire HDV, and that your camera is actually outputting HDV and not DV downconverted from HDV.
What framerate did you shoot? You also have to match your framerate in the Video Input Preference -
I think you will not gain too much from upgrading to 8.2.3. And since you said that your system currently is extremely stable I an not sure if I can recommend the upgrade without this suggestion:
What I would do is to get another harddrive, either an external FireWire drive or an internal one. Then clone your startup disc (e.g. with carbon copy cloner), install the new OS and the Trial versions of Red, plus V8.2.3a. Give it a long test run to see what you would gain from this setup. But seriously, if you want a real performance and productivity boost you have to think about a new Mac and e.g. Media 100 Suite with a Kona LHe board or a Producer system.

Here you can find QuickTime 7.5 for 10.4 and 10.5 (on the right side of the screen).

You only need QuickTime 7.5.5 for the ProApps Update 2008-03, which only contains Compressor 3.0.4 and Qmaster 3.0.4. If you do not need these two tools at 3.0.4 but you need FCP 6.0.4 you either can install the ProApps update 2008-02, which does contain FCP6.0.4. Unfortunaltey it is not available as a download anymore, but maybe you still have it somewhere on your HD? Or you can manually install FCP 6.0.4 from the ProApps Update 2008-03. When downloading this update from the Support website (not from Software Update), right-click on the installer Package and click “Show Package content”. There are the individual installers for FCP 6.0.4 and the other apps. Most of them you can install withouth Quicktime 7.5.5
