I think Mark is dead on with this one, but I wanted to add a couple more possibilities to his first two.
In Chicago land I am seeing the same issues with the local spots on Comcast you are (wherever you are) Bottom-line Comcast has consolidated head ends, and eliminated any sort of Quality Control. I honestly believe that at this point there is literally one or two guys in Colorado who monitor all of the hundreds of channels around the country on the Comcast machine. I have literally seen channels off the air for days because of transponder or other satellite failures with a slate that reads “please switch to such and such transponder” and just try to call the 800 number and try explaining that to the guy in India. Hell CNBC had a major lip sync issue for 2 years! (It was fine on the dishes)
The other possibility is that I have heard that the production departments in the head ends only perform QC on spots produced in house by them. So that means the terrible used car spot they produced ends up looking better on air then the one your client actually spent some money having you produce. Good luck explaining that one to your client!
Comcast blows and I really hope they disappear soon, I don’t feel to bad though since they never bothered to unhook the cable at my new house when I moved in 5 years ago or even bother to do an audit of the neighborhood!

Pricewise, Bob is leading you in the right direction with the Blackmagic recommendation, it appears their new fiber tx/rx’s are probably the cheapest available at this point.

Wiring Massive Building for Video/Audio Sends!Richard is right, if you are thinking of pushing HD in the future, I would consider fiber if it’s in your budget. You can get “Cubes” so all you would do is run fiber at your patch panels, and plug in the cubes when you want to use that location.

2 things to try. One is to bring your own separate riser or three apple boxes and set up behind him over his shoulder. The other is to add a sandbag to the middle of your tripod (or hang it if you have one with a hook under the head) just make sure it’s not TOO heavy.

Make up some multicore cables, or get a multidyne Lightbox if you have to run over 300′, it’s much cheaper and easier.

To answer Joel’s original question, “No Joel Premiere didn’t kill Vegas, SONY DID!”
I have been using Vegas since almost the very beginning. Every version promises more, and comes up very short. This last version has me finally fed up enough to switch completely. I just spent a lot of money to upgrade my system to 64 bit to take advantage of all of the new features and abilities that Sony claims 9.0 has. Just about every damn thing I do causes Vegas to crash! This system has been checked out by multiple people, I have rebuilt it and reinstalled Vegas multiple times, E-mailed Sony MANY times with no helpful response. I finally decided to upgrade to CS5, buy a Quadro card, and damn is it sweet!
Now don’t get me wrong, I really miss the idea of editing Vegas, but at the end of the day I, like you, can’t afford to use something that is going to constantly crash, fail to live up to it’s promises, create stress and cause setbacks, all of which costs me money…Buy a Quadro and give CS5 a chance!

It also has on screen audio meters as well. They don’t replace traditional metering equipment, but they are good for confidence monitoring.

Ant, I am using a Panasonic BT-LH2600W that I found used on E-bay. The great thing about is that with 2 inputs you can monitor the multi. & program, and it also has a built in WFM. I know they are a little expensive but you can find a couple of them on E-bay right now. I have also used the HD-SDI to HDMI converter from AJA with a consumer Plasma monitor and it produced a great picture (It’s about half the price of the extra output card.
