Premiere can handle all kinds of resolutions in one sequence.
SD will suffer qualityloss in a HD timeline.
You can try AE (latest version)which can upscale the SD footage now with good results.

I do not use a mac so no idea how idvd works,
but I do know dvd only holds 29,97 fps.
You probably burned a data dvd instead of a video-dvd.
For a video-dvd you need authoring software like Encore.

You cannot select standard or widescreen.
Premiere will capture the footage how it was recorded to tape.
If the footage is anamorfic widescreen it will be captured in 4;3 but Premiere will interprete the footage as widescreen.

First make a sequence that matches your footage.
If there is no preset you can make a custom one.

The green bands could be a grafics card driver issue.
