Offline (Cutting copy) Vanishing bug

I don’t know if this is a bug or a simple something I am doing to cause this issue.
When I tab about and wander away from the conform page, I frequently return to my Offline, seemingly, vanished from my source viewer. I say vanish and not unlinked because I can still view it on my external monitor, and as a reference wipe in Color. A simple reboot of the program has been my work-around for the last year, but I’m finding some projects it happens almost every instance I move away from the conform page and return. There isn’t “one thing” I can point to that I have done. Sometimes it is a simple tab back and forth that accomplishes this nuisance.
I’m on 9.1.6 dongled.
Many thanks comrades if you can provide some bug spray. (or a slap to the head)
