It’s probably shot in HD anamorphic and requires you to set the correct pixel aspect ratio. So the file is in 4×3 but the actual image is 16×9 squeezed into that.

Is your USB audio interface getting enough juice from the USB? Have you tried testing it with integrated audio?

Try changing the data levels from the clip attributes. They might be interpreted incorrectly.

Prores is available under Quicktime in the latest version of Premiere.

That’s not a video format. That’s a media player. If you don’t know what format you’re working with you can use the free MediaInfo application to see what it is.
