check if smoothing is set to ‘ON’ for every vector type.

restart Illustrator and it should be smooth now

on mask layer you can use gradient tool and use black/white or black/transparant for a more even fade-in effect.

if it only occurs in PS, just reset the preferences.
also check file setup

which Profile are you using?

adobe 1998 is a more common one.

or copy past all layers in new document.

opening a new file using a different pixel aspect ratio should fix it as well i guess

SHIFT+ F8 is the shortcut in Illustrator for shape tool
