Hi,
I would recommend using the following filters:

– Color Correction Hue/Saturation (for the green color)
– Stylize Mosaic (for the low resolution, pixelated look, this is optional depending on the kink of image in the monitor)
– Sapphire Scanlines Mono (for the scanlines)
– distortion Bulge at 0.3 height or so (to acchieve the roundness of old monitors)

after this you could add a slight glow.
