I’ve never heard of the missing transcode files problem. Never has happened to me.
No, you don’t need an AUDIO_TS folder. That’s an urban legend (which was that some very early first generation players required it), but this has never been proved, and the DVD-Video spec doesn’t mention it. And if it was required, it’d be in the spec.

You don’t need an AUDIO_TS folder unless you are authoring to the DVD-Audio format. Encore authors to DVD-Video only, and DVD-Video requires only a VIDEO_TS folder, not an AUDIO_TS.
Parent folder = containing folder. In other words, if you want to add a file to the ROM content, you put it in the folder, and then choose the folder in Encore for the ROM content. If you want folder1 full of files to be added to the ROM content, you put folder1 inside folder2, and choose folder2 for your ROM content.

That is correct. But there should have been a PDF on the disc at the root level.
If you want to include a folder in the ROM content, then you must put the folder in a parent folder, then select the parent folder for your ROM content in Encore.

DVD-Video allows up to 99 chapters per title set (timeline), and up to 99 title sets (timelines).

If there’s a reproducible way to get Encore into this state in the first place, I’d be interested in the details.

If your audio preferences are set to Dolby Digital, then yes, Automatic will transcode your audio to AC3.
Encore only offers bitrate control over AC3 files.

If you’re not using Chapter Index Menu (this is in the “Menu” menu, then you don’t need to worry about this at all. Just leave all buttons as Normal.
