Look in Control Panel>System>Hardware>Device Manager>Sound, Video and Game Controllers>Video Codecs>Properties.
Go through the list of installed codecs. Cinepak, Indeo and Microsoft should make up the bulk of the list. If you have a lot of strange-looking codecs installed, then post some of them here and we should be able to tell whether or not they are the likely source of your difficulties with Encore.

Do you have DivX or any codec packs installed? Encore doesn’t play nice with DivX or codec packs.

I didn’t realize that *all* of the chapters had moved. Encore 1.x has a slight problem with previewing chapters that are imported from Premiere Pro in the Monitor window – they sometimes show up in the wrong place (by about 15 frames). The good news is that even though they preview incorrectly, they get placed correctly when the disc is burned.
Even if you didn’t import your chapter markers from Premiere Pro, this still may be a preview-only issue. Try this: once your chapter points show up 15 frames off, burn another disc right away and see if the new burned disc has the chapter markers in the right place.

Is the override for the timeline set to the main menu? If it is, change the override setting to “Not Set”.

I suspect this is a media/burn speed issue. First of all, Memorex and Fuji are crap. There’s no nice way to put it. You get what you pay for. Secondly, the faster you burn, the greater chance of introducing imperfections in the burn that will be seen as errors by cheaper and less-capable players. My personal rule is to never burn 16x media any faster than 4x. 8x media goes off at 2x.
IMHO, if you use quality media (Taiyo Yuden, Verbatim) and use slower burn speeds, your disc compatibility rate will go way up.

Yes. Encore’s automatic transcoding does a great job.

How close is the trimmed point to either the original beginning of the clip or the next chapter point? There needs to be at least a half-second between them (about 15 frames).

Automatic transcode settings in Encore are moving targets, based on the amount of video material in a project. It’s not permanently set to 4MB or 7MB or anything else.
Are you possibly referring to the default preset that appears when you open the Transcode Settings dialog?

Are you trimming an mpeg file that has already been transcoded? There may be an issue with where the closest GOP to your trim point is in the transcoded file. AFAIK Encore can only trim to I-frames at the beginning of GOPs.
